Registration Form

• Older 4’s and 5 year olds, for children eligible for kindergarten, but need another year of preschool. It is also open to children turning 5 by December 30, 2008. The class meets 4 days per week M-TH from 12:45 pm to 3:45 pm.

•  4 year olds, for children who turn 4 by September 30, 2008. The class meets 3 days per week M, W, F in the morning from 8:45 am to 11:45 am or in the afternoon from 12:45 pm to 3:45 pm.

• 3 year olds, for children who turn 3 by September 30, 2008. This class meets 2 days per week T, TH in the morning from 8:45 am to 11:45 am or in the afternoon from 12:45 pm to 3:45 pm.

•2 ½ year olds, for children that will be turning 3 by March 31, 2009. The class meets 1 day per week on Wednesday or Friday morning from 9:00 am to 11:30 am.  Children must be toilet trained.  Sibling discount does not apply to this class.  This class meets in the classroom in the church building.

• Our morning classes run from 8:45 am to 11:45 am. The 2 ½ year old class runs from 9:00 am to 11:30 am.  All afternoon classes run from 12:45 pm to 3:45 pm.

• We open the week of September 8, 2008 with an Open House/Meet Your Teacher and will run until June 2, 2009. 

• All children must be toilet-trained.

• Tuition must be paid in total or in 9 equal monthly payments by the first of every month.

• A 20% discount applies to the lower tuition rate(s) when 2 or more children from the same family are enrolled.  This does not apply if one child is enrolled in the 2 ½ year old class.

• At the time of registration a non-refundable registration fee of $100 is required.

• Submission of the registration form does not guarantee enrollment.  If none of your choices is available you may choose to place your child on a waitlist.  The registration fee will be collected when a spot becomes available for your child and you choose to enroll him/her at St. David’s. If you are on the waitlist and have not received placement by the beginning of the school year, you must contact the school to remain on the waitlist.
